Records & Privacy

The Aspen Group staff members keep names, medical conditions, treatments and family information private. Access to confidential information by staff members is limited to the information necessary to perform their job duties and responsibilities. Any Aspen Group staff member that violates the agreement with The Aspen Group or any federal or state statute or regulation will be subject to disciplinary action, up to and including dismissal and/or legal sanctions. Information is not released unless appropriate authorization and/or consent is obtained as described in our Privacy Notice.

The Aspen Group will create a chart for each patient containing all physician's orders, clinical notes and pertinent medical information. Staff members use this chart to record patient treatments and medical conditions. A patient can ask to see such information through an appropriate request as described in the Privacy Notice.

Problem Solving Procedure
The Aspen Group protects patients' rights. However, any concern or problem can be expressed by contacting the manager or clinical supervisor at the patient's local Aspen Group branch. Additionally, you may call The Aspen Group's Corporate Office, 1-208-529-0800, to report problems or concerns with service.
