Transfer & Discharge

Circumstances sometimes arise that result in a patient's transfer, referral or discharge from The Aspen Group's services. Some of these situations include, but are not limited to:

  • The patient's medical condition changes
  • The patient's living area becomes unsafe
  • The patient does not comply with a physician's orders

The Aspen Group will inform the patient, family member or caregiver prior to a patient's discharge (except in case of emergency). The Aspen Group will provide information regarding other agencies and assist the patient in changing care if a transfer is required. In such an instance,The Aspen Group will inform the chosen agency of what is necessary for the patient's continued care. Upon discharge, information will be provided to assist in further patient care.
